Wooster School-Based Health Center

We would like to invite families to join the Wooster Middle School School-Based health Center (Health Haven). Health Haven offers a wide variety of valuable services to Wooster students through our Nurse Practitioner and Social workers. Services include diagnosis and treatment of illnesses (ex. cold, allergies, headache, flu, etc.) and injuries (ex. sprains, concussions, etc.) sports physicals, immunizations, individual and group counseling, crisis intervention, and referrals to specialty care. Special groups focus on such topics as nutrition, bullying prevention, 7th grade transition and girls' health. Parents and students view this well-established program as an important benefit to Wooster Middle School.

Without Health Haven, many students would experience barriers to care such as high costs, deductibles, lack of insurance, transportation issues, and scheduling problems. Health Haven eliminates these barriers by providing medical and mental health services on-site during school hours, regardless of a student's ability to pay. We do not collect co-pays. We encourage families who are covered by their own insurance plans to enroll their children, even if they only use Health Haven to administer a Tylenol now and then. Be aware that these services are not available without the family making a decision to join Health Haven by filling out the enrollment form.

If your child is entering 8th grade and is already enrolled in Health Haven, please read this important message: In order to maintain accurate, up-to-date medical and insurance information we ask that you fill out an enrollment form for this current school year. Updated forms allow us to provide the best care possible for your child.

Please complete all pages of the enrollment form, sign, and return it at your earliest convenience to the address or fax# below or have your child deliver it to the Health Haven office. If the enrollment form is returned without a signature, we will not be able to treat your child.
